Position:  RN CASE MANAGER-HOSPICE

 

Dept:  Home & Community Based Services

Reports to:  RN Clinical Manager-Hospice

Hours:  Full-Time, 60hrs/pp, primarily 1st shift, on-call rotation required, eligible for all benefits

Job Summary: This position will utilize clinical judgement and assessment skills while providing nursing care to our hospice clients in their homes on St. Camillus Campus and off campus in the community.

Qualifications:

  • Current WI RN License and current CPR certification.
  • BSN preferred.
  • Min. 3 years medical-surgical experience as an RN, 2+ years Hospice experience preferred.
  • Professional communication skills.
  • Transportation to and from clients’ homes; when utilizing a personal vehicle, a valid license and current liability insurance coverage as well as a driving record which complies with the Safe Driving Policy.

 

Position:  LPN/RN

 

Dept:  Skilled Nursing

Reports to:  Director of Nursing

Wage:  $21-25.30/HR(LPN) ,  $25.75-31.00/HR (RN)

Hours:  FT days, 64hrs/bi-weekly, 6:30-3pm, E/O weekend.  (All Benefits)

                  Weekends only position, Days 6:30a-6:30p, Every Sat/Sun (All Benefits)

Job Description:

  • Provides skilled nursing care and assists RN to meet the physical, spiritual and psychosocial needs of the residents.
  • The staff LPN responds in emergency situations to assure resident safety and security.
  • Medication Administration
  • Assists with resident care and does treatments(i.e., transcriptions of Dr. Orders, ordering of labs, setting up appointments)
  • Documents care per policy and procedures.
  • performs tasks related to the general supervision of the CNA staff.

Requirements:

  • Current WI LPN/RN license, graduate of an accredited school of nursing.
  • Minimum 1 year professional nursing experience, preferably experience in a LTC facility.
  • Current CPR Certification
  • Accurate data entry skills and familiarity with electronic health records.

 

Position:  RN SUPERVISOR

 

Department:  Skilled Nursing

Reports to:  Director of Nursing

Hours:  POOL POSITIONS, hours vary, 2 weekend shifts/month requirement, 2 holidays/yr.  ($37/HR)

Responsibilities:  Oversees nursing care provided on the shift including assisting nurse managers in evaluating staff, assessing residents, making daily rounds, delegating assignments to staff and other duties as assigned.

Requirements:  Current WI RN license, 2+yrs related experience preferred.  Strong clinical/supervisory skills & knowledge of federal/state regulations, accurate data entry skills and familiarity with electronic health records.  Current CPR certification.

 

Position:  LPN SUPERVISOR/CBRF ASSISTED LIVING

 

Hours:  FT noc's, 48-64hrs/bi-weekly, 10p-6:30a,  E/O weekend (All benefits)

Starting range:  $23-27.70/HR plus shift differentials for pm's & noc's

Job description:

  • Provides support and oversight to Residential Aides, I.e., coaching, evaluations, communication regarding resident care
  • Provides assistance with medications, sets up, follow up with Physician, conduct audits
  • Assists in treatments and services, reports and documents appropriately
  • Assists in the resident services plans, attends weekly meetings

Requirements:

  • Graduate of an accredited school of nursing, Current State of WI licensure as an LPN
  • Community-Based Residential Facilities (CBRF)-certified as per State of WI Administrative Code DHS 83 requirements
  • Current CPR Certification
  • Ability to work independently with minimal supervision
  • 1-2 years experience as a supervisor in a CBRF setting preferred
