Position:  RN-START OF CARE

 

Dept:  Home & Community Based Services

Reports to:  Clinical Manager-Home Health

Hours:  Typically 1st shift, per visit, average about 10 hours/week with potential to grow.   Willingness to admit on some weekends

Job Summary:  This position will utilize safe judgement and assessment skills, while coordinating, implementing, and developing a plan of care for clients on and off of St. Camillus campus.  Off campus admissions are typically within a 20-mile radius of St. Camillus.  This position will be responsible for:

  • Opening new Home Health client cases
  • Coordinating with other members of the interdisciplinary healthcare team for care needs
  • Department and campus wide facility meetings as assigned.

Qualifications:

  • Current WI RN License and CPR certification
  • BSN preferred
  • Minimum of 3 years med-surgical experience as an RN and 2 years of Home Health experience preferred
  • Previous electronic documentation experience preferred.
  • Ability to work independently as well as strong time management skills.
  • Strong communication skills.
  • When utilizing a personal vehicle to travel to and from client homes, a valid license and proof of current liability insurance coverage, as well as a driving record which complies with the St. Camillus Safe Driving Policy.

 

Position:  LPN/RN

 

Dept:   Skilled Nursing

Reports to:  Director of Nursing

Hours:   FT days, 80hrs/bi-weekly, 6:30a-3pm, E/O weekend.  (All benefits)

                    FT pm's, 64hrs/bi-weekly, 2:30p-11pm, E/O weekend.  (All benefits)

                    Noc Baylor, work 32 hrs/bi-weekly, get paid for 40hrs.  One wknd 4 hr shifts, 6:30p-10:30p, one wknd 12 hr shifts 11p-7a (All benefits

                   except Health)

                   PT pm's, 48hrs/bi-weekly, 2:30p-11p, E/O weekend.  (All benefits except Health)

                   Pool positions available: $27/HR Requirements: 2 weekend shifts/month & 2 Holidays/yr. All shifts available for pool.

Job Description:  Provides skilled nursing care and assists RN to meet the physical, spiritual and     psychosocial needs of the residents.  The staff LPN responds in emergency situations to assure resident safety and security.

Requirements:   Current WI LPN license, graduate of an accredited school of nursing.  Minimum  2 years professional nursing experience, preferably experience in a LTC facility.

 

Position:  LPN SUPERVISOR/CBRF ASSISTED LIVING

 

Hours:  Pool positions available: Requirements:  2 weekend shifts/month  & 2 Holidays/yr.   All shifts available for pool.

                  PT days, 48hrs/bi-weekly, 6:30a-3pm

Job description:

  • Provides support and oversight to Residential Aides, i.e., coaching, evaluations, communication regarding resident care
  • Provides assistance with medications, sets up, follow up with Physician, conduct audits
  • Assists in treatments and services, reports and documents appropriately
  • Assists in the resident services plans, attends weekly meetings

Requirements:

  • Graduate of an accredited school of nursing, Current State of WI licensure as an LPN
  • Community-Based Residential Facilities (CBRF)-certified as per State of WI Administrative Code DHS 83 requirements
  • Current CPR Certification
  • Ability to work independently with minimal supervision
  • 1-2 years experience as a supervisor in a CBRF setting preferred
