Wage:  $17.85-21.50

Hours:  FT pm's 64hrs/bi-weekly, 2:30p-11pm, E/O wknd (Benefit eligible)

Job Description:

  • Provides skilled nursing care and assists RN to meet the physical, spiritual and psychosocial needs of the residents.
  • The staff LPN responds in emergency situations to assure resident safety and security.
  • Medication Administration
  • Assists with resident care and does treatments(i.e., transcriptions of Dr. Orders, ordering of labs, setting up appointments)
  • Documents care per policy and procedures.
  • performs tasks related to the general supervision of the CNA staff.

Requirements:

  • Current WI LPN/RN license, graduate of an accredited school of nursing.
  • Minimum 1 year professional nursing experience, preferably experience in a LTC facility.
  • Current CPR Certification
  • Accurate data entry skills and familiarity with electronic health records.

 

Position:  RN CASE MANAGER

 

Dept:  Home & Community Based Services

Hours:  Typically 1st shift, per visit, part-time with more hrs as we grow!

Job Summary: This position will be responsible for opening new Home Health client cases, developing & implementing care plans, case management, supervision of LPN’s and HHA’s, conducting regular client home visits, & coordinating with other members of the interdisciplinary healthcare team.

Qualifications:

  • Current WI RN (BSN preferred) & CPR certification
  • Minimum of 3 yrs med-surgical experience as an RN, 2 yrs Home Health experience preferred
  • Ability to work independently.
  • Strong time management & communication skills.
  • Strong computer skills required; previous electronic documentation experience preferred.
  • Current WI driver’s license, a good driving record, and the ability to travel to clients’ homes within 30-mi radius.

 

Position:  On-Call RN

 

Department:  Hospice & Home Health

Job Highlights:

  • Allows for work from home taking clinical calls for clients after hours and weekends in rotation w/the other on call staff.
  • Provides clinical coverage when necessary to clients on St. Camillus campus and off-site in the community.

The right candidate should have:

  • Current WI RN license (BSN preferred) and current CPR certification.
  • 2 years medical-surgical experience as a Registered nurse.
  • Previous experience in Home Health and Hospice preferred.
  • Transportation to and from clients’ homes.

 

Position:  RN SUPERVISOR

 

Department:  Skilled Nursing

Reports to:  Director of Nursing

Hours:  Baylor position,  Work Every Sat/Sun 8 hour shifts 6:30a-3pm 32hrs/bi-weekly and get paid for 40hrs/bi-weekly!  (Benefit eligible for all except health insurance)

Responsibilities:  Oversees nursing care provided on the shift including assisting nurse managers in evaluating staff, assessing residents, making daily rounds, delegating assignments to staff and other duties as assigned.

Requirements:  Current WI RN license, 2+yrs related experience preferred.  Strong clinical/supervisory skills & knowledge of federal/state regulations, accurate data  entry skills and familiarity with electronic health records.  Current CPR certification.

 

Position:  CLINICAL NURSE MANAGER

 

Department: Jesuit Community-Home & Community Based Services

Hours:  Full Time; 80hrs/pp 1st shift; eligible for all benefits including health

As the RN Clinical Manager, one will:

  • Oversee the day-to-day functions of the staff and residents in the unlicensed/non-regulated Jesuit Community with varying acuity and independence.
  • Collaborate with our team of talented nursing staff to design a state of the art healthy aging program utilizing innovation and creativity.
  • Work with numerous outside community partnerships such as universities, hospitals, and other organizations to pilot innovative aging programs for the residents.
  • Quality 1 on 1 time with the residents of the Jesuit Community; allowing for rapport and relationship building.
  • Lead the nursing team to provide compassionate, quality care aligned with the non-profit, faith-based mission and values of St. Camillus.

Qualifications:

  • Current Wisconsin RN License; graduate of an accredited school of nursing.
  • Minimum of 5 years’ supervisory experience preferably in a regulated long term care facility.
  • Current CPR certification
  • Strong verbal/written communication using tact, diplomacy and discretion with staff, members of the health care team, families and residents.

 

Position:  LPN SUPERVISOR/CBRF ASSISTED LIVING

 

Hours:  FT pm's, 64hrs/bi-weekly, E/O weekend and rotating holidays

Starting range:  $23-27.70/HR plus shift differentials for pm's & noc's

Job description:

  • Provides support and oversight to Residential Aides, I.e., coaching, evaluations, communication regarding resident care
  • Provides assistance with medications, sets up, follow up with Physician, conduct audits
  • Assists in treatments and services, reports and documents appropriately
  • Assists in the resident services plans, attends weekly meetings

Requirements:

  • Graduate of an accredited school of nursing, Current State of WI licensure as an LPN
  • Community-Based Residential Facilities (CBRF)-certified as per State of WI Administrative Code DHS 83 requirements
  • Current CPR Certification
  • Ability to work independently with minimal supervision
  • 1-2 years experience as a supervisor in a CBRF setting preferred
