Our Hospice Team

Medical Services

St. Camillus Hospice has a Medical Director who is responsible for the overall supervision of the medical aspects of the Hospice Care recipient’s Plan of Care. The Medical Director works in collaboration with the individual’s attending physician, as well as with the individual and their family, in order to meet the specific needs of the Hospice Care recipient.

The Medical Director serves as an educator and resource for the members of the Hospice Care Interdisciplinary Team.

Nursing Services

Nursing services within St. Camillus Hospice are provided by registered nurses, licensed practical nurses and hospice aides. The Plan of Care is developed, initiated and directed by the Hospice RN. All members of the Hospice Care recipient’s nursing team are collaborative members of the Interdisciplinary Team.

Social Worker

Social work services of St. Camillus Hospice encompass a broad range of care needs for both the individual and their family. The social worker plays a key role in the care planning process and is a vital member of the Interdisciplinary Team.

Volunteer Services

A volunteer’s contributions are diverse and dependent upon the Plan of Care developed for the Hospice Care recipient. Volunteers may be drawn upon for socialization, emotional support, running errands, shopping, respite or clerical duties.

Chaplain

A St. Camillus chaplain provides spiritual support to the individual and their family. At the request of the family, they may facilitate communication with the individual’s personal clergy in the community.

The Plan of Care

The Wholistic Plan of Care is the tool that drives the type and scope of compassionate attention that a person needs while receiving Hospice Care. The plan is a highly individualized, dynamic and integrative process that is developed by all Hospice Care Team Members: doctors, nurses, social workers, chaplains, aides and volunteers. Ancillary team members may include:  physical, occupational, and speech therapists, and dieticians.

The needs of the Hospice Care recipient and his or her family are of the utmost concern. Members of the team are committed to delivering the very best practices to the continuum of end of life care. Always with dignity, integrity and respect. To every person, everyday.
